If you are having trouble opening the form, please click here and be sure to include your Organization, Event, Brief Description Of Event, Talent’s Role At Event, Event Date, Time, Time-Frame Commitment (Start/End Time), Audience Size and Composition, Event Location, Attire, Program Agenda, Others Participating, Requester Name, Requester Contact Information (phone, address, email, role), and name of talent you are requesting.
